Failure 1: Starting with Technology Instead of a Problem
This is the most common failure, and it kills more AI projects than any technical limitation.
How It Happens
Someone in leadership reads an article about AI. Or a vendor gives a compelling demo. Or a board member asks "what is our AI strategy?" The company decides it needs to "do AI" and spins up a project to explore the technology.
The team evaluates tools, runs experiments, builds a proof of concept that shows a capability. But when they try to connect that capability to a real business outcome, they cannot. The technology works, but nobody has a clear problem it solves better or cheaper than existing methods.
Six months and EUR 50,000-200,000 later, the project stalls. The team gets reassigned. AI is labeled "not ready for us."

The rule: If your first AI project takes more than 6 weeks to deliver its first measurable result, the scope is too large. Cut it in half. Then cut it in half again.
Failure 3: Perfect Data Syndrome

Failure 5: Building Instead of Buying (or Vice Versa)
How It Happens
The build trap: A company with a strong engineering team decides to build everything from scratch. Custom models, custom infrastructure, custom everything. The project takes 6x longer than expected and costs 10x more, because building production-grade AI systems is significantly harder than building a prototype.
The buy trap: A company purchases an off-the-shelf AI solution that promises to solve their problem out of the box. It sort of works, but not well enough for their specific context. They spend months trying to customize a tool that was not designed for their use case, paying for features they do not need while lacking features they do.

The right approach is usually: assemble. Use pre-built components where they exist and add custom logic where your business is unique.
For most MKB AI projects, the stack looks like:
- Pre-built: AI models (OpenAI, Anthropic, Google), automation platforms (n8n, Make)
- Custom: Prompts, workflows, business rules, integrations with your specific systems
- Managed: Hosting, monitoring, security
You do not need to train a custom AI model to process invoices. You need a well-designed workflow that uses an existing model with prompts tailored to your invoice formats.
The rule of thumb: If a pre-built tool handles 70% of your requirements, use it and customize the remaining 30%. If it handles less than 50%, evaluate whether a different tool is a better fit before building from scratch.
Failure 6: Ignoring Change Management

The technical implementation is flawless. The AI system works beautifully in testing. Then it gets deployed to real users and nobody uses it.
Why? Because the team that is supposed to use the system was not involved in its design. They do not trust it. They do not understand it. They see it as a threat to their jobs. Or they have perfectly rational concerns about edge cases that the system does not handle.

Involve end users from day one. Not as passive reviewers, but as active participants in the design:

The AI project launches. Everyone agrees it seems to be working. But nobody can quantify the impact because baseline metrics were never established.
Was the old process really taking 20 hours per week, or was that an estimate? Are we actually processing invoices faster, or does it just feel that way? Did customer satisfaction improve, or are we imagining it?
Without measurement, the AI project cannot demonstrate its value. Without demonstrated value, it cannot secure budget for maintenance, improvements, or expansion. It slowly degrades and eventually gets abandoned.

Establish baseline metrics before the project starts. For every expected benefit, measure the current state:
